The effect of selenium supplementation on oxidative stress, clinical and physiological symptoms in patients with migraine: a double-blinded randomized clinical trial

Background and AimThe present double-blinded randomized clinical trial aimed to investigate the effect of selenium supplementation on oxidative stress, clinical, and physiological symptoms in patients with migraine.MethodsIn total, 72 patients with migraine were randomly assigned to receive either 200 μg/day selenium (n = 36) or placebo (n = 36) for 12 weeks. Clinical traits of migraine (e.g., severity, frequency, and duration of headaches), mental health indices (e.g., depression, anxiety, and distress), quality of life, biomarkers of oxidative stress (e.g., nitric oxide [NO], malondialdehyde [MDA], total antioxidant capacity [TAC], total oxidant status [TOS]), and anthropometric indices were assessed at baseline and at the end of the study.ResultsSelenium supplementation resulted in a significant reduction in NO (−1.24 ± 0.43 vs. 0.16 ± 0.43; p = 0.03) levels and a significant increase in TAC (9.89 ± 2.50 vs. −0.18 ± 2.50; p = 0.01) compared to the placebo group. Moreover, selenium supplementation had a significant protective effect against MDA levels compared to placebo (0.33 ± 0.57 vs. 1.83 ± 0.57; p = 0.03). In addition, selenium intake was associated with a lower headache frequency (−8.15 ± 0.77 vs. −4.12 ± 0.77; p < 0.001) and severity (−2.89 ± 0.42 vs. −1.16 ± 0.42; p = 0.01) as well as a lower Headache Impact Test-6 (HIT-6) score (−9.22 ± 2.00 vs. −2.08 ± 2.00; p = 0.02) compared to the controls. For other outcome variables, we found no significant effect.ConclusionSelenium supplement may be considered a complementary therapy in patients with migraine due to its beneficial effects on oxidative stress and migraine symptoms. INTRODUCTION: This study aimed to evaluate and raise the therapeutic hypothesis of the use of low-level laser as a complementary therapy in patients with sleep disorder (SD) in a broad approach involving other pathologies superimposed on this pathology. METHODS: This is a clinical case report of a patient with Parkinson's disease (PD) complaining of sleep disorder. The proposed treatment is the use of photobiomodulation (FB) with intraoral application using an innovative dosimetry. SD was assessed using an adapted analogue scale. RESULTS: In the evaluation performed after the last session, there was an improvement in the sleep disorder compared to the pre-treatment evaluation. However, one month after the end of the treatment, it was observed that the quality of sleep remained practically the same. CONCLUSION: We can conclude that low-potency FB can be a modulator in the production of neurotransmitter substances in a local and systemic way, being a complementary or alternative treatment for patients with sleep disorder (SD).

The Effect of Benson's Relaxation Technique on Fatigue and Quality of Life of Patients Diagnosed With Heart Failure.

Patients with heart failure (HF) reported poor quality of life (QOL) due to different reasons among which fatigue is the most important. Improving QOL is a crucial objective for patients with HF and their primary health care providers. Managing fatigue with medication is not enough. Benson's relaxation technique (BRT) is a complementary therapy used to manage fatigue among different populations with limited studies checking its effect among patients diagnosed with HF. The purpose of this quasi-experimental study was to check the effect of BRT on fatigue and QOL among 140 (68 intervention and 72 control) patients diagnosed with HF. Intervention group performed BRT for 20 minutes twice a day for 2 months. Control group received regular care from their health care providers. At baseline, there were no differences between intervention and control groups regarding fatigue, physical component summary, and mental component summary. At follow-up, intervention group had higher scores in physical and mental component summaries than control group (45.48 ± 10.52 vs 37.97 ± 14.78) and (46.22 ± 8.39 vs 41.01 ± 10.36), respectively. Also, intervention group had lower levels of fatigue than control group (2.54 ± 0.87 vs 6.33 ± 0.61). In conclusion, the use of BRT as a complementary therapy for patients with HF might decrease fatigue level and improve QOL.

Inositol and herbal substances as elements of complementary therapy in patients with PCOS

Introduction: Polycystic ovary syndrome is a common hormonal disorder in women and the leading cause of female infertility. Traditional therapeutic methods often do not yield the expected results, hence the need to explore new treatment strategies. Promising results have been observed in studies on inositol. Objective: The aim of this study is to summarize the current knowledge regarding the effectiveness and safety of inositol and herbal preparations in the treatment of PCOS based on available scientific literature. Materials and Methods: A literature review was conducted using PubMed and Google Scholar databases, using search terms: inositol, PCOS treatment, herbal medicine in PCOS. Current knowledge: Inositol is an oral supplement used in the therapy of PCOS. It is characterised by high safety and minimal risk of side effects. Herbal extracts alleviate symptoms in patients with PCOS. Conclusions: Analysis of scientific research has provided evidence of the effectiveness of inositol, particularly myo-inositol, in complementary therapy for PCOS. Herbal medicine also appears to be an effective supportive treatment. However, these substances should not be considered as primary therapy but rather as an adjunct. Only both - primary medications and complementary treatment methods, can yield a therapeutic effect.

Terapi Komplementer dalam Mengatasi Gangguan Tidur Pasien yang Menjalani Hemodialisis

This study aims to collect relevant information on the effectiveness of complementary therapies in treating sleep disturbances in patients undergoing routine hemodialysis. The method used in this study is a literature study by collecting and reviewing several published articles regarding the role, use, and effect of complementary therapy in patients on routine hemodialysis with sleep disturbance problems. The results showed the effectiveness of complementary therapies such as music therapy, acupressure, and Benson's relaxation in overcoming sleep disturbances and improving patients' sleep quality. In conclusion, the three complementary therapies can overcome sleep disturbances and improve hemodialysis patients' sleep quality. However, of the three therapies, the authors concluded that Benson's relaxation intervention was an intervention that had better effectiveness than other complementary therapies.

The Effect of Lower Urinary Tract Symptoms on Quality of Life and Complementary Therapies for Patients with Multiple Sclerosis

Multiple sclerosis patients face many physiological, psychological, and social problems depending on the type and course of the disease. One of the physiological problems is lower urinary tract symptoms (LUTS). Problems such as urinary incontinence, frequent urination, sudden urination, inability to urinate, inability to empty the bladder completely affect the daily life of the patients. These patients may avoid social interaction because of frequent urination and incontinence. Patients with multiple sclerosis refrain from shopping, meeting with friends, and other social activities due to urinary problems. In addition, they also restrict their fluid intake because they urinate frequently. Urinary tract infections associated with low fluid intake and inability to urinate are more common in multiple sclerosis patients than in the normal population. These problems restrict the patient’s life and negatively affect their quality of life. These complaints can be minimized by bladder training, pelvic floor muscle exercises, nerve stimulation, or use of urinary catheter in order to eliminate these problems and improve the quality of life of the patients. While bladder training prolongs the urination intervals of patients, pelvic floor muscle exercises reduce urinary incontinence by strengthening the muscles. Neuromodulation reduces these problems by suppressing nerve impulses. Catheters are important for patients who cannot urinate or have urinary incontinence. Urinary incontinence, retention, and urinary infection can be reduced by using catheters.

Expanding the potential therapeutic options of hemoperfusion in the era of improved sorbent biocompatibility.

Hemoperfusion has been considered a promising adjuvant treatment for chronic diseases and some acute states when specific removal of pathogenic factors from the bloodstream is desired. Over the years, advances in adsorption materials (e.g., new synthetic polymers, biomimetic coating, and matrixes with novel structures) have renewed scientific interest and expanded the potential therapeutic indications of hemoperfusion. There is growing evidence to suggest a prominent place for hemoperfusion as an adjuvant treatment in the setting of sepsis or severe coronavirus disease 2019 and as a therapeutic option for chronic complications associated with accumulated uremic toxins in patients with end-stage renal disease. This literature review will describe the principles, therapeutic perspectives, and the emerging role of hemoperfusion as a complementary therapy for patients with kidney disease.

Coronary sinus Reducer device for the treatment of refractory angina: A multicenter initial experience

IntroductionThe coronary sinus Reducer (CSR) device has emerged as a complementary therapy in patients with severe angina refractory to optimal medical therapy and not amenable to revascularization. Our aim was to assess the safety and efficacy of the CSR in a real-world setting. MethodsTwenty-six patients with refractory angina (RA) and evidence of myocardial ischemia attributable to the left coronary artery considered unsuitable for revascularization were treated with the CSR at two centers between May 2017 and July 2019. Safety endpoints were procedural success and complications. Efficacy endpoints, assessed at six-month follow-up, were reduction in CCS class, improvement in quality of life (QoL) assessed using the short version of the Seattle Angina Questionnaire (SAQ-7), and reduction in antianginal therapy. ResultsTwenty-three patients had end-stage coronary artery disease without revascularization targets and three had microvascular disease without epicardial stenosis. Procedural success was achieved in 23 patients, with two device/procedure-related complications and one anatomically-related failure to deliver the device. A total of 24 patients had the device implanted and entered the efficacy analysis. Eighteen patients (75.0%) had a reduction of at least one CCS class, 41.7% had a reduction of at least two classes, and 16.7% became asymptomatic, with a mean reduction in CCS class of 1.3±0.2 (p=0.001) at six-month follow-up. All SAQ-7 domains improved, notably physical limitation (p=0.001), angina frequency (p=0.005) and QoL (p=0.006). There was a mean reduction in anti-ischemic drugs from 3.4±1.1 to 2.9±1.2 (p=0.010). ConclusionIn this real-world, multicenter experience, implantation of the CSR was associated with improvement in angina and QoL in patients with RA.

Efficiency of complementary integrative practices in patients with Parkinson’s disease

Introduction: Parkinson’s disease is a disorder that causes damages to brain cells responsible for producing dopamine at the encephalic region known as black matter, a chronic and progressive disease which has as symptom hand trembling, as well as slowness and stiffness of movement and loss of balance, symptoms that compromises the daily life of the patient. The treatment is generally administered through medication. Another course to treat Parkinson’s disease is through Integrative and Complementary Practices, also known as non-pharmacological therapies, which are regarded as traditional practices for preventing and reducing symptoms related with the pathology. Objective: This study has as its goal to verify the efficacy in the application of Integrative and Complementary Practices to decrease symptoms in patients with Parkinson’s Disease. Methodology: This is an exploratory, descriptive study, with a qualitative approach with integral literature review, analyzed from PubMed and SCIELO databases. There were selected 05 articles for review. Results: The study demonstrates that, though the benefits of medication and other procedures administered to treat the disease are well known, the complementary therapies also contribute to improve the motor and non-motor symptoms of the patient. It was observed that Yoga, Relaxation, Music Therapy, Acupuncture and Anma Massage Therapy all resulted on improvement for the patient. Conclusion: Therefore, it is concluded that the usage of complementary therapies in patients with Parkinson’s disease is efficient and aids in the motor and mental development of an individual.

Clinical effects of proprioceptive thumb exercise for individuals with carpometacarpal joint osteoarthritis: A randomized controlled trial

Study DesignRandomized Control Trial. IntroductionThumb CMC joint OA is a common diagnosis. Currently there is no evidence available to under if proprioceptive neuromuscular training is an effective intervention for this population. Purpose of the studyTo establish the effectiveness of a proprioceptive training program as a complementary therapy for patients with thumb CMC joint OA. MethodsStandard conservative thumb CMC joint OA treatments were received by both the control (n = 26) and experimental groups (n = 26) for a period of 12 weeks. The experimental group received a proprioceptive training program during the same intervention period. Outcome measures included severity of pain with activity according to the numerical rating scale (NRS), QuickDASH, Canadian Occupational Performance Measure (COPM), and proprioception via joint position sense (JPS). ResultsFifty-two females participated in the study. Both the experimental and control group made both clinically and statistically significant changes in the mean VAS and COPM scores over time. Only the experimental group achieved a statistically and clinically significant change in JPS error score over time. DiscussionExperimental group achieved a statistically significant change in JPS over time in concordance with previous investigations. Changes in pain scores differed from prior investigations and the between-group comparison was not statistically significant. Changes in the Quick DASH was similar to previous findings. ConclusionProprioceptive training in addition to a traditional rehabilitation program decreased error scores on the joint position sense test.

Virtual Reality Applications in Chronic Pain Management: Systematic Review and Meta-analysis

BackgroundVirtual reality (VR) is a computer technology that immerses a user in a completely different reality. The application of VR in acute pain settings is well established. However, in chronic pain, the applications and outcome parameters influenced by VR are less clear.ObjectiveThis review aimed to systematically identify all outcome parameters that are reported in relation to VR in patients with chronic pain.MethodsA total of 4 electronic databases (PubMed, Scopus, Web of Science, and Embase) were searched for relevant studies. Multilevel random-effect meta-analyses were performed, whereby the standardized mean difference was chosen as the effect size to denote the difference between measurements before and after a VR intervention.ResultsThe initial database search identified 1430 studies, of which 41 (2.87%) were eventually included in the systematic review. Evidence has been found for the effects of VR on pain, functioning, mobility, functional capacity, psychological outcomes, quality of life, neuropsychological outcomes, and physical sensations. The overall effect size (a total of 194 effect sizes from 25 studies) based on a three level meta-analysis was estimated at 1.22 (95% CI 0.55-1.89; z=3.56; P<.001), in favor of improvements after a VR intervention. When categorizing effect sizes, the overall effect sizes were reported as follows: 1.60 (95% CI 0.83-2.36; z=4.09; P<.001) for the effect of VR on pain (n=31), 1.40 (95% CI 0.13-2.67; z=2.17; P=.03) for functioning (n=60), 0.49 (95% CI −0.71 to 1.68; z=0.80; P=.42) for mobility (n=24), and 0.34 (95% CI −1.52 to 2.20; z=0.36; P=.72) for functional capacity (n=21).ConclusionsThis systematic review revealed a broad range of outcome variables influenced by an intervention of VR technology, with statistically significant pain relief and improvements in functioning. Terapi Komplementer Terhadap Tekanan Darah pada Penderita Hipertensi: Studi Literatur

Background. Hypertension or high blood pressure is an abnormal increase in blood pressure in the arteries, transporting blood from the heart and pumping it throughout the tissues and organs of the body continuously for more than one period. The use of complementary therapy has been widely used to accelerate the recovery of patients with hypertension. The purpose of this study was to analyze studies related to complementary therapy in hypertensive patients.&#x0D; Methods. The literature search was carried out in 3 databases, namely Google Scholar, SINTA and Pubmed. The research taken is research published in the last 10 years. The design of this study used the literature review method. The population of this study were all national and international journals related to complementary therapy in patients with hypertension. The sample in this study was 10 articles from international journals.&#x0D; Results. Based on the results of the search for journals, it can be concluded that, complementary therapy in this study found 5 types of approaches. These approaches include meditation, murotal, exercise, cucumber herbs, and cupping therapy.&#x0D; Conclusion. It is hoped that this literature study can provide additional information for health workers regarding the types and effectiveness of complementary therapies in patients with hypertension. Moreover, the results of this study can be used for further research to conduct more representative research.

Meditation: Evidence Map of Systematic Reviews

Study Basis: This evidence map presents a summary of studies that addressed the effects of meditation on various clinical and health conditions. Meditation is a contemplative practice that has been used for the promotion of health, and the treatment of different conditions.Method: The study is based on the search of four electronic databases for the period 1994-November 2019 and includes systematic reviews, meta-analyses, meta-syntheses, and integrative reviews. 3iE evidence gap map was the methodology of choice, and AMSTAR 2 was used for the analyses. Tableau was used to graphically display the confidence level, number of reviews, health outcomes, and intervention effects.Results: This map encompasses 191 studies, with Mindfulness being the key word that retrieved the highest number of results. Several meditation techniques were evaluated in different contexts, and the confidence levels of 22 studies were high, 84 were moderate, and 82 were low. Two 2 meta-syntheses and 1 integrative review were also included. Most of the studies reported positive effects and a beneficial potential of the practice of meditation. Health outcomes were divided into five groups out of which mental health and vitality, and well-being and quality of life stood out with the largest number of studies.Conclusions: Meditation has been applied in different areas. This Evidence Map intends to be an easy visual tool to access valuable evidence-based information on this complementary therapy for patients, health professionals, and managers.

Effect Of Complementary Therapy On Hypertension Patients: Systematic Review

The research was conducted using the method literature review. The most effective use of therapy among the elderly is therapy using herbal medicine because herbal therapy is easy to reach and cheap and can even be obtained free of charge around the neighborhood. Ameliorating Effects of Coriander on Gastrocnemius Muscles Undergoing Precachexia in a Rat Model of Rheumatoid Arthritis: A Proteomics Analysis.

Coriander is a commonly used vegetable, spice, and folk medicine, possessing both nutritional and medicinal properties. Up to two-thirds of patients with rheumatoid arthritis (RA) exhibit loss of body mass, predominately skeletal muscle mass, a process called rheumatoid cachexia, and this has major effects of the quality of life of patients. Owing to a lack of effective treatments, the initial stage of cachexia has been proposed as an important period for prevention and decreasing pathogenesis. In the current study, we found that cachexia-like molecular disorders and muscle weight loss were in progress in gastrocnemius muscle after only 5 days of RA induction in rats, although rheumatoid cachexia symptoms have been reported occurring approximately 45 days after RA induction. Oral administration of coriander slightly restored muscle loss. Moreover, iTRAQ-based quantitative proteomics revealed that coriander treatment could partially restore the molecular derangements induced by RA, including impaired carbon metabolism, deteriorated mitochondrial function (tricarboxylic acid cycle and oxidative phosphorylation), and myofiber-type alterations. Therefore, coriander could be a promising functional food and/or complementary therapy for patients with RA against cachexia.

Selective Toxicity Effect of Fatty Acids Omega-3, 6 and 9 Combination on Glioblastoma Neurons through their Mitochondria.

Glioblastoma (GBM) is one of the most common malignant tumors of the central nervous system that occurs in the brain and is a deadly disease. Despite the different approaches to the treatment of this malignancy, the discovery of new compounds with anti-cancer effects seems necessary. In this study, the selective toxicity effects of omega 3, 6 and 9 combinations on mitochondria isolated from U87MG human glioma cells and also human embryonic kidney 293 cells (HEK293) as normal control were investigated. The results indicated that the omega 3, 6 and 9 combinations significantly reduced succinate dehydrogenase (SDH) activity only in mitochondria isolated from U87MG human glioma cells. Additionally, exposure of mitochondria isolated from U87MG human glioma cells to this combination was associated with a selective increase in the level of reactive oxygen species (ROS), the collapse of the mitochondrial membrane potential (MMP), mitochondrial swelling and cytochrome c release. However, these effects were not observed in mitochondria isolated from HEK293 cells (as a normal group). According to results, it is proposed that the combination of omega 3, 6 and 9 could induce toxicity in U87MG human glioma cells through their mitochondria. This combination can be helpful as a complementary therapy in patients with GBM.

Factors associated with the use of complementary therapies in Taiwanese patients with systemic lupus erythematosus: a cross-sectional study

BackgroundThis study aimed to investigate the prevalence of and the factors associated with the regular use of complementary therapies for Taiwanese patients with systemic lupus erythematosus (SLE).MethodsIn this cross-sectional study, 351 patients with SLE were consecutively recruited from a regional hospital in southern Taiwan from April to August 2019. Demographic and clinical information, including the use of different types of complementary therapies, was ascertained using a self-constructed questionnaire. Disease-specific quality of life was measured using the Lupus Quality of Life (LupusQoL) questionnaire. SLE disease activity was assessed using the rheumatologist-scored Systemic Lupus Erythematosus Disease Activity Index 2000 (SLEDAI-2 K). Factors associated with the regular use of complementary therapies were evaluated using multiple logistic regression analyses.ResultsOf the 351 patients with SLE, 90.3% were female, and 60.1% were ≥ 40 years of age. The prevalence of the regular use of any type of complementary therapy was 85.5%. The five most popular types of complementary therapy used were (1) fitness walking or strolling, (2) Buddhist prayer or attending temple, (3) vitamin consumption, (4) calcium supplementation, and (5) fish oil supplementation. Multiple logistic regression analyses revealed that the significant and independent factors associated with the regular use of complementary therapies in patients with SLE were age ≥ 40 years (adjusted odds ratio [aOR] 2.76, p = 0.013), nonoverweight or nonobesity (aOR 0.29, p = 0.004), engagement in vigorous exercise in the past year (aOR 4.62, p = 0.002), a lower SLEDAI-2 K score (aOR 0.90, p = 0.029), and a lower score in the physical health domain of the LupusQoL (aOR 0.57, p = 0.001).ConclusionsA high prevalence of complementary therapy use in Taiwanese patients with SLE was observed. Rheumatologists should routinely ask patients about their use of supplements to minimize the risk of interaction with medical therapy.

The Effectiveness of Acceptance and Commitment-Based Therapy on the Immune Function and Activity Limitations in Patients With Irritable Bowel Syndrome

Introduction: Irritable bowel syndrome is the most common, costly, and disabling dysfunction of the gastrointestinal tract. Mental disorders can be one of the main factors in the onset, continuation, or exacerbation of gastrointestinal signs and symptoms in people with this syndrome. Commitment and acceptance therapy is a mixture of four approaches of awareness, acceptance, commitment, and behavior change, and its overall goal is to achieve psychological flexibility to move towards thought-based behavior. This treatment focuses less on reducing symptoms and more on improving the quality of life. Objective: This study aimed to determine the effectiveness of acceptance and commitment-based therapy on the immune function and activity limitations in patients with irritable bowel syndrome. Materials and Methods: This study was a quasi-experimental research with a pre-test, post-test design and a control group. Thirty patients with irritable bowel syndrome were randomly selected from those referred to a hospital clinic in Tabriz City, Iran. They were randomly divided into the intervention and control groups (15 in each group). The intervention group received acceptance and commitment group therapy for 8 sessions, while the control group did not receive any intervention. Both groups were assessed before and after the intervention, and in the follow-up phase using the activity limitations subscale of quality of life questionnaires (to measure activity limitations) and stool calprotectin test to assess safety performance. Data analysis was performed using descriptive statistics indices (mean and standard deviation) and analysis of variance with repeated measures, Bonferroni test, and analysis of covariance to test the study hypotheses. Results: The Mean±SD age of the intervention group was 34.53±18.38 years, and the Mean±SD age of the control group was 42.80±17.97 years. The significance levels of Levene’s and the Shapiro-Wilk test for all variables were greater than 0.05. Based on the results, the Mean±SD score of the activity limitations variable in the intervention group was 7.53±4.24 in the pre-test, 17.66±3.41 in the post test, and 17.26±3.65 in the follow-up. According to the results, activity limitations (P=0.027) and safety performance (P=0.034) were significantly different before and after the intervention. Conclusion: The present study’s findings show that acceptance and commitment-based therapy can play an influential role in improving patients’ immune function and promoting their activity limitations. Therefore, this treatment is recommended as a complementary therapy in patients with irritable bowel syndrome.
